Job summary

McGregor PACE is seeking a Community Resource Liaison to connect participants with local resources and services, helping them stay healthy and independent in the community. You will work with the interdisciplinary team under MSW/LSW supervision, coordinating housing, social supports, and access to services while maintaining confidential records and following up with partner agencies.

• Experience in community outreach or social services preferred.
• Strong interpersonal and writing skills.
• Knowledge of local community resources.
• Ability to work independently and in a team.

Responsibilities

• Coordinate and follow up with community agencies, healthcare providers, social service organizations and other service partners.
• Support participants with housing, social support, and other needed services.
• Help identify barriers to care and communicate with MSW/LSW.
• Assist with scheduling and coordinating community-based services.
• Maintain accurate, timely and confidential participant records.
• Perform other duties as assigned by the MSW/LSW.
