About the Role

Our client is seeking an experienced and motivated Senior Community Health Worker to lead and support vital health initiatives within the community. As a Senior member of the team, you will also mentor junior health workers and contribute to program development and evaluation. Key Responsibilities

• Provide comprehensive health education and counseling to individuals and groups on various health topics, including disease prevention, nutrition, and chronic disease management, through virtual platforms.

• Conduct remote health screenings and follow-ups to monitor client progress and adherence to care plans.

Requirements

• A relevant qualification or certification in Community Health Work, Public Health, Nursing, or a related field.
• Minimum of 5 years of experience working as a Community Health Worker or in a similar public health role.
• Demonstrated experience in health education, client counseling, and navigating healthcare systems.
• Excellent verbal and written communication skills, with the ability to convey complex health information clearly.
• Proficiency in using virtual communication tools (video conferencing, phone) and electronic health record systems.
• Strong organizational and time management skills, with the ability to manage a caseload remotely.
• Experience in mentoring or supervising junior staff is highly desirable.
