Job summary

Qualifications

Responsibilities

• Conduct comprehensive assessments including history, meds, vitals, and exam.
• Support members during a short-term sub-acute rehab episode.
• Ensure accurate documentation including ICD-10 coding.
• Collaborate with interdisciplinary care team.
• Conduct advanced illness and advanced care planning discussions.
• Provide counseling and education to patients and caregivers.
