Job summary

VCU Health in Richmond, VA is seeking a Clinical Social Worker 3 (MSW) to provide direct patient and family care and help coordinate team-delivered services within the care plan.

The role includes psychosocial assessments, treatment planning, discharge coordination, patient/family education, and community referrals, with Virginia licensure preferred and a minimum of 3 years of clinical social work experience in a specialty area.

Qualifications

• Master of Social Work Degree from a Council on Social Work Education accredited program.
• Licensed Clinical Social Worker in Virginia preferred.
• CSW Level 3: minimum of three (3) years of clinical social work experience with at least one (1) year in a specialty practice area.

Responsibilities

• Complete psychosocial assessment and develop treatment plans aligned with the overall health care plan.
• Provide psychosocial intervention; educate patients, their family members, staff and social work students; facilitate compliance with treatment.
• Coordinate discharge planning activities and community referrals.
• Initiate education activities with facility staff.
• Contribute to quality improvement activities through self-analysis of services and needs.
• Participate in service area meetings, training, and patient care conferences.
