Qualifications

• Current LCSW license required.
• Minimum of five years of psychiatric social work experience.
• Experience in case management and discharge planning preferred.
• Experience providing individual, group, and family therapy preferred.
• One year in a lead role preferred.

Responsibilities

• Conduct clinical biopsychosocial assessments covering mental health symptoms, housing, finances, and safety.
• Develop and revise interdisciplinary plans of care addressing social and psychiatric factors.
• Provide individual, group, and family therapy to patients.
• Implement therapeutic interventions and coordinate resources for complex issues.
• Collaborate daily with families, the care team, and community agencies to ensure safe discharge.
