Job summary

Saint Luke’s Health System is seeking a full-time Pharmacist to care for solid organ transplant patients. The pharmacist will dispense medications, monitor regimens and collaborate with interdisciplinary teams in a busy hospital setting.

The position requires Missouri/KS pharmacist licensure and experience with transplant pharmacy; PGY1/PGY2 is a plus. Day shift with weekend and evening rotation accompanies a comprehensive patient-care focus.

Qualifications

• Licensed pharmacist with MO/KS regulatory credentials.
• Transplant experience preferred but not required.
• MO Pharmacist, MO MTS, and KS Pharmacist are required

Responsibilities

• Prepare and dispense medications and monitor therapy.
• Collaborate with interdisciplinary teams and supervise support staff.
• Design, implement and adjust therapy plans to achieve patient goals.
• Provide drug information and education to patients and caregivers.
• Document changes in the EMR and discuss orders with prescribers.
• Participate in safety programs for adverse events and medication errors.
