Job summary

Alder, Inc. is expanding its Business Development team in Fairfield County, CT and Dutchess County, NY, hiring a Senior Client Liaison.

You will build trusted relationships with hospitals, SNFs, and home health partners, translating relationships into consistent referrals while collaborating on market strategy with the Director of Strategic Partnerships. This field-based role requires experience in healthcare development, strong communication, and the ability to work independently.

Qualifications

  • 3–5 years in healthcare business development, provider relations, or referral roles.
  • Experience with home health, rehab, or related clinical settings preferred.
  • Comfort talking with frontline staff and leadership; organized and proactive.

Responsibilities

  • Build relationships with hospitals, SNFs, and rehab partners.

  • Position Alder as a trusted post-discharge and care navigation partner.
  • Develop referral pathways with home health agencies and visiting nurse associations.

  • Represent Alder at community events and clinical meetings.
  • Track activities and referrals; document in HubSpot; share trends with Growth Team.

The Role:

We’re expanding our Business Development team and hiring a Senior Client Liasion to strengthen Alder’s referral presence across hospitals, skilled nursing facilities, home health agencies, and select physician practices throughout Fairfield County (CT) and Westchester County (NY).

This role is ideal for someone with hands‑on experience in healthcare business development or provider relations who understands how referrals truly flow within clinical environments. You’ll build practical, trusted relationships with both frontline and leadership teams—and translate those relationships into consistent, appropriate referrals to Alder.

You’ll work closely with our Director of Strategic Partnerships, collaborating on market strategy while you focus on the clinical channels that require day‑to‑day presence, education, and follow‑through. This is a field‑based role with real ownership and a direct impact on Alder’s growth in Fairfield County (CT) and Dutchess County (NY).

Responsibilities:

Build relationships with hospital, SNF and rehab partners

  • Manage outreach and ongoing engagement with hospitals, skilled nursing facilities, inpatient rehab, and sub‑acute providers
  • Establish Alder as a trusted post‑discharge and care navigation partner.

Engage home health and VNA leadership

  • Build referral pathways with home health agencies, visiting nurse associations, and outpatient therapy providers.
  • Position Alder as a complementary partner—not a competitor—in supporting complex patients and families.
  • Build relationships with select physician specialties (e.g., primary care, geriatrics, neurology) where care coordination needs are high.
  • Clearly communicate Alder’s role, referral criteria, and value to both clinicians and office staff.

Represent Alder in the community

  • Attend relevant clinical meetings, in‑services, and local events.
  • Serve as a credible, professional representative of Alder’s clinical and service standards.

Track activity and results

  • Document partner interactions, referrals, and follow‑ups in HubSpot.
  • Share insights on referral trends, barriers, and opportunities with the broader growth team.
Required skills / experiences:
  • 3-5 years of experience in healthcare business development, provider relations, clinical partnerships, or referral-based roles.
  • Background in home healthcare, visiting nurse services, outpatient PT, rehab, or related clinical settings strongly preferred.
  • Comfort operating in clinical environments and speaking with both frontline staff and leadership.
  • Highly organized, proactive, and comfortable working independently in the field.
  • Strong communication skills with a practical, relationship‑first approach.
  • Mission‑driven mindset and interest in improving care transitions for older adults and families.

In accordance with the New York and Connecticut Pay Transparency Acts, the expected salary range for this position begins at $80,000 annually, with the opportunity for commission and bonus. Actual pay will vary depending on factors such as licensure, years of relevant experience, and additional qualifications.
