Responsibilities

• Facilitates identification and resolution of gaps in care from hospitalization, at discharge, and into ambulatory care setting.
• Leads development and implementation of complex care plans with defined goals with interprofessional team and patient/family input.
• Promotes evidence-based practice through journal club and practice changes; supports continuing education and knowledge dissemination.
• Evaluates patient and family experience, adjusts care to improve satisfaction and quality of care.
• Safety-sensitive position; maintain alertness and safe performance at all times.
