Qualifications

• RN license required.
• Nursing diploma or associate degree required; bachelor preferred.
• 2-5 years experience; 5-10 years preferred.

Responsibilities

• Identify and resolve gaps in care from hospitalization to discharge and into ambulatory care.
• Lead development and implementation of complex care plans with goals in collaboration with the interprofessional team and patient/family.
• Promote evidence-based practices and advance nursing practice through learning and standards updates.
