TSINY, a mental health services agency is seeking a Senior Case Manager for its Bronx, NY Residential Program.
Driver’s license required
The Senior Case Manager coordinates and monitors the assessment, treatment planning and care coordination for each resident on their caseload. The Senior Case Manager serves as an advocate and intermediary for and on behalf of residents’ rights. The Senior Case Manager will work in partnership with our residents to establish goals, find appropriate supports and access independent community housing.
Bachelor’s degree plus one year’s experience, Associate’s degree plus two years’ experience or High School diploma plus three years’ experience in mental health setting.
