Job summary

Connections Health Solutions is seeking a Case Manager III in Kirkland, WA, to facilitate admission intakes, plan treatment, manage crises, and coordinate care with the interdisciplinary team. The role carries a caseload and requires an LMHP/Licensed credentials in Washington with 3+ years of mental health experience.

Full-time, on-site with comprehensive benefits. The position offers salary range corresponding to local market with shift differentials and a commitment bonus for full-time staff.

Qualifications

• Master’s degree in behavioral health, Social Work, Counseling, Psychology, or related discipline.
• Licensed mental health professional in WA (LMHP, LICSW, LMHC, LMFT or associate).
• At least 3 years of experience in mental health or substance abuse treatment.
• Knowledge of the WA State behavioral health system and resources.
• Must meet vaccination policy and credentialing requirements.

Responsibilities

• Facilitates admission intakes, treatment planning, and discharge planning.
• Coordinates with interdisciplinary treatment team to develop individualized Treatment Plans.
• Provides case management services to promote recovery and community reintegration.
• Maintains clinical records and documents services accurately.
• Participates in supervision and ongoing education to maintain competency.
