Job summary

Connections Health Solutions in Kirkland, WA is seeking a Case Manager III to coordinate admission intakes, treatment planning, discharge planning, and crisis intervention within an interdisciplinary team. You will carry a caseload and ensure timely assessments and person-centered plans.

Qualified candidates hold a Master’s in behavioral health or related field, WA licensure, and at least 3 years of mental health experience.

Qualifications

• Master’s degree in behavioral health, Social Work, Counseling, Psychology, or related discipline.
• Unrestricted WA license (LMHP/LicSW/LMHC/LMFT or associate) required or in process.
• At least 3 years of experience in mental health or substance abuse treatment.

Responsibilities

• Conduct psychosocial assessments with individuals in a timely manner.
• Review and sign off on assessments by licensed staff when appropriate.
• Educate on consents and authorizations for treatment and disclosures.
• Participate in interdisciplinary treatment team meetings and develop personalized treatment plans.
• Coordinate care, discharge planning, and referrals with internal and external providers.
• Maintain case management records, including documentation of services and interventions.
