Job summary

Qualifications

• Bachelor’s Degree in Social Work or Human Service field and two years direct care experience preferred; or equivalent.
• Valid Driver’s License and acceptable driving record.

Responsibilities

• Conduct assessments to accept, deny, and refer applicants.
• Verify and document eligibility and maintain client service records.
• Assist clients in locating, securing and retaining housing.
• Develop housing stability plans with clients and provide long-term case management.
• Coordinate services and crisis response as needed; provide self-advocacy coaching.
• Establish and maintain relationships with landlords, referral sources and community resources.
• Travel to client homes, shelters, and workplaces as required.
