Job summary

Heartland Family Service in Council Bluffs, IA seeks a Case Manager for the Heartland Homes program. This role provides long-term case management and supportive services to families, using a Housing First approach, from two facilities in Council Bluffs.

You will assess eligibility, document records, and collaborate with the Property Manager to support tenancy, finances, and access to benefits. 25 hours weekly, evenings or weekends, $19.74–$23.54/hr.

Qualifications

• Bachelor’s Degree in Social Work or Human Service field; two years direct care experience preferred.
• Valid Driver’s License with acceptable driving record.
• Experience applying trauma-informed care and crisis intervention techniques.

Responsibilities

• Assist daytime Case Manager with assessments to accept, deny and refer applicants.
• Verify eligibility and maintain comprehensive service records for clients.
• Assist clients in identifying strengths, needs and tenancy obligations.
• Help clients access and maintain employment or benefits.
• Provide or refer crisis response services to meet household needs.
• Coach clients to develop communication and self-advocacy skills.
