SHELTER, Inc. is seeking a Senior Case Manager for the Beck Avenue Navigation Center, a 138-bed facility providing wraparound services to unhoused participants. You will deliver mental health support and coordinate access to medical, psychiatric, social, educational, and other services.

The role aims to optimize functioning and resiliency by linking participants to natural supports and evidence-based care. Strong caseload management and collaboration are essential.

Qualifications

• Master’s degree in Social Work or related field required.
• License as ACSW/LCSW/LMFT/LPCC highly desired.
• Minimum 2+ years of trauma-informed clinical case management or counseling.

Responsabilités

• Screen and assess participants for eligibility and risk.
• Develop personalized treatment plans and monitor progress.
• Coordinate housing, medical and social services for participants.
• Maintain HMIS and case files with timely updates.
• Ensure confidentiality and comply with required reporting.
• Collaborate with staff and external partners for wraparound services.
