Qualifications

Responsibilities

• Oversee all services and operational activities.
• Provide comprehensive case management including screening, assessment and individualized service plans.
• Crisis intervention and connection to health, mental health, employment, housing and community resources.
• Foster a professional, welcoming environment with clients, staff and partners.
• Manage and evaluate the performance of assigned staff.
• Maintain accurate documentation and progress reports per guidelines.
• Coordinate life management classes and arrange speakers/workshops as needed.
• Ensure compliance with federal and state guidelines and standards.
• Participate in weekly case staffing and promote professional growth.
