Job summary

LifeLong Medical Care is seeking a Case Manager IV to join the primary care team in Berkeley. The role conducts outreach, engages patients with complex needs, develops patient-centered care plans, and leads Enhanced Care Management implementation across sites.

Responsibilities include home visits or clinic-based care, coordinating referrals, assisting with benefits access, housing navigation, and data-informed caseload management in a culturally responsive, team-oriented environment.

Qualifications

• High school diploma or GED required for the role.
• Bachelor’s degree in Social Work, Health or Human Services preferred.
• Experience in community-based health care or social work settings is desirable.
• Proficient with Microsoft Office suite.

Responsibilities

• Conduct patient outreach and engagement to assess needs.
• Develop and support patient-centered care plans.
• Coordinate referrals and assist with applications for benefits.
• Maintain caseload and use data to support program requirements.
• Provide housing navigation and crisis intervention when needed.
• Document services in required data systems (EHR, HMIS).
