Job summary

American IV seeks a compassionate Wellness Director (RN/LPN) to lead clinical care in our Phoenix senior living community. You will oversee resident assessments, care plans, and medication management for assisted living and memory care.

You will supervise nursing and caregiving teams, ensure regulatory compliance, and collaborate with families and physicians to support resident wellbeing in a faith-inspired culture of integrity and service.

Qualifications

• RN licensure or LPN licensure required. Nursing leadership experience in senior care preferred.
• Experience with geriatric care and memory care populations.
• Experience in resident assessments, care planning, and medication management.
• Proven ability to supervise and schedule nursing and caregiving staff.
• Familiarity with EHR systems and regulatory documentation.
• Knowledge of infection control practices and quality audits.
• Excellent communication with families, physicians, and interdisciplinary teams.

Responsibilities

• Lead wellness programs and oversee clinical care for residents in assisted living and memory care.
• Supervise, mentor, and schedule nursing and caregiving staff to ensure high-quality care.
• Conduct resident assessments, care planning, and ongoing health evaluations.
• Coordinate with physicians, families, and interdisciplinary teams on resident needs.
• Oversee medication management, documentation, and regulatory compliance.
• Promote a faith-inspired, resident-centered culture of dignity, safety, and respect.
• Develop and monitor infection control and quality improvement initiatives.
• Provide training, coaching, and performance feedback to care team members.
