Job summary

Your Health Organization in the Savannah area is seeking a Senior Solutions Advisor to help patients and families navigate aging, chronic illness, and care transitions. The SSA connects people to appropriate healthcare, community services, and privately paid caregiving options, supporting education, relationship building, and community integration.

The role travels to patients' homes and facilities daily, is full-time, and typically follows an 8-hour workday (8:00 AM–5:00 PM).

Qualifications

• Bachelor’s degree or equivalent in a related field is preferred.
• Strong interpersonal and communication skills with the ability to build trust with families and providers.
• Self-starter with time management and coordination abilities.

Responsibilities

• Meet with patients and families to assess long-term care needs and social support challenges.
• Provide consultative guidance on living arrangements, safety concerns, and available Your Health services.
• Serve as the regional point of contact for onboarding new patients.
• Maintain knowledge of local and regional senior care resources including facilities and services.
• Act as liaison to ensure seamless access and coordination of resources.
• Educate community professionals, family caregivers, and providers through outreach and training.
• Timely communication with all new patients in the assigned Care Group or Care Team.
• Support care transitions by working with Transitional Care Providers and care teams.
• Assist families in accessing care management and hospice services.
• Track and document activities for billing (CCM, ACP, etc.).
• Participate in community events and health fairs to promote SSA services.
• Collaborate with leadership to support market expansion through referrals.

Education

Bachelor’s Degree in Social Work, Human Services, Healthcare Administration, or related field
