Senior Care Services Coordinator

Job summary

ENOA is seeking a Services Coordinator to determine and monitor long-term care needs for financially eligible clients over 65 who require home and community based services. You will coordinate services, identify strengths and needs, and support consumer-directed options to promote safe, independent living.

Responsibilities include comprehensive assessments, planning, implementation, and advocacy to ensure quality outcomes within Nebraska’s Medicaid framework.

Qualifications

• Baccalaureate degree in human services, education or health/medical, or RN with Nebraska license and 2+ years in long-term care or related field.
• Experience with aging/gerontology and psycho-physiological traits preferred.

Responsibilities

• Report to work as scheduled on a regular basis.
• Complete required pre-service orientation and training.
• Maintain confidentiality of client information per HIPAA regulations.
• Coordinate assessments, planning and service delivery for eligible participants.
• Travel across the five-county area to identify needs and coordinate services.
