Job summary

Sagility is hiring a Senior Care Specialist (Registered Nurse) to work from home. In this seasonal role, you will apply clinical expertise to support members with chronic conditions, conducting assessments by phone and developing care plans.

You will coordinate services, educate members, and collaborate with a multidisciplinary team to improve health outcomes. The assignment ends June 2027, with potential bonuses based on performance and business needs.

Qualifications

• Active RN license in the state of residence.
• Active Compact RN license for applicable states.
• 2 years of clinical nursing experience; 3+ preferred.
• Strong clinical judgment, critical thinking, and problem-solving.
• Proficient in Word, Excel, and Outlook; able to navigate EMR systems.
• Remote work ability and independent work ethic.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and group health plan members.
• Develop actionable, individualized care plans based on member needs and identified health risks.
• Establish health coaching relationships with members and provide education and support related to chronic condition management.
• Use nursing judgment to assess member needs and coordinate appropriate interventions for acute situations.
• Support transitions of care and safe discharge planning, including coordination of DME, home health services, provider appointments, and other necessary resources.
• Help members and caregivers navigate healthcare benefits, services, and available care options.
• Assess physical, environmental, and psychosocial factors that may affect a member's health and overall well-being.
• Collaborate with multidisciplinary teams, including social workers, dietitians, pharmacists, providers, and other healthcare professionals.
• Promote member self-management by providing education and resources appropriate to individual health conditions and goals.
• Coordinate care and condition-management activities among members, caregivers, providers, and other members of the care team.
• Document member interactions, assessments, care plans, interventions, and outcomes accurately and in a timely manner within applicable systems.
