Job summary

Sagility is seeking experienced and compassionate Registered Nurses to join our team as Senior Care Specialists in a work-from-home capacity. This seasonal position carries an assignment end date of June 2027 and may include bonuses and incentives based on business needs.

Qualifications

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license allowing practice in applicable states.
• Minimum 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical thinking and problem-solving skills.
• Ability to work independently in a remote environment.
• Intermediate to advanced computer proficiency including Word, Excel and Outlook.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, and measurable care plans based on member needs and risks.
• Establish health coaching relationships and provide education and support for chronic condition management.
• Use nursing judgment to assess needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning, including DME and home health services.
• Navigate healthcare benefits and care options for members and caregivers.
• Assess factors affecting health and well-being; collaborate with multidisciplinary teams.
• Promote self-management with education and resources tailored to health conditions.
• Coordinate care activities among members, caregivers, providers, and care team.
• Document interactions, assessments, and outcomes accurately in systems.
