Job summary

Ascension Living in Des Plaines, IL seeks a dedicated Registered Nurse for Nazarethville Place to provide direct nursing care in a long-term care setting. You will implement care plans, monitor patient conditions and coordinate with the healthcare team for well-coordinated care.

You’ll ensure safety, carry out physician orders, and assist with discharge planning. Minimum requirements include RN licensure and BLS certification within 30 days.

Qualifications

• RN licensure and BLS within 1 month of hire
• Diploma from accredited school of nursing or equivalent licensure at time of hire
• Experience in long-term care is a plus

Responsibilities

• Provide direct nursing care to residents in a long-term care setting.
• Implement and monitor individualized care plans.
• Monitor and communicate patient conditions to the care team.
• Coordinate with physicians, therapists, and nursing staff for well‑coordinated care.
• Carry out physician and nursing orders accurately.
• Identify safety hazards and respond to emergencies to ensure resident safety.
