Job summary

ArchWell Health is seeking a Market Quality Coordinator to drive STARS and HEDIS performance in the Phoenix market. You will partner with center leadership to close quality gaps and improve patient outcomes in a senior-focused primary care setting.

The role involves patient outreach, medical record review, and coordination of in-center screenings while maintaining HIPAA standards and collaborating with market leaders to remove barriers and optimize care delivery.

Qualifications

• Working knowledge of quality measures and quality gap closure strategies.
• Experience working in a primary care setting.
• HIPAA confidentiality and data security.
• Ability to work independently with attention to detail.
• Demonstrated teamwork and effective communication.

Responsibilities

• Develop and implement strategies to close STARS gaps.
• Review medical records to document and close Focus On Care measures.
• Request and upload medical records to EMR system.
• Conduct patient outreach calls to support gap closure and scheduling of screenings.
• Schedule and perform ancillary visits for in-center screenings.
• Meet with Market Leaders to address roadblocks and plan initiatives.
• Ensure OSHA compliance and proper documentation at all times.
• Take on additional duties as needed.
