Job summary

Qualifications

• Certified PTA with IL licensure and good standing.
• Experience in skilled nursing or post-acute care preferred.
• Ability to work under PT supervision and document care.

Responsibilities

• Assist seniors to improve mobility, function, and quality of life.
• Document care in EMR and maintain accurate records.
• Collaborate with interdisciplinary care team.
