Job summary

UPMC Central PA is seeking full-time Adult and Senior Care Physicians to join our Post-Acute Care team across multiple skilled nursing facilities in the Hanover region. Qualifications

• Board certified or board eligible in Primary Care, Internal, Family or Geriatric Medicine.
• Geriatrics-focused care with a patient-centered approach.
• Collaborative team player with APP supervision experience.
• Eligible for licensure in Pennsylvania.

Responsibilities

• Provide direct patient care across skilled nursing, rehab, personal care/assisted living, independent living and clinic.
• Collaborate with facility leadership, discharge teams, care management and other specialties to drive quality care.
• Supervise Advanced Practice Providers and coordinate care across facilities.
• Utilize EPIC EMR and PACS for online diagnostics and imaging results.
• Clinical rounding M-F with flexible schedules; evening and weekend call shared among partners and APPs.
