Job summary

Penn State Health Post-Acute Care is seeking a full-time Board Certified/Board Eligible Physician to join our team serving rehabilitation and long-term care patients in suburban Lancaster County, PA. You will provide comprehensive medical care at one, and no more than two, trusted long-term care communities.

The role offers clinical autonomy within a collaborative environment, with a focus on high-quality patient relationships and outcomes, and a schedule that balances daily rounds and dedicated

Qualifications

• MD, DO, or foreign equivalent required.
• Completed ACGME-accredited residency program.
• BE/BC in Geriatrics, Internal Medicine, or Family Medicine.
• Ability to obtain Pennsylvania medical license.

Responsibilities

• Provide comprehensive medical care at one (or two) post-acute care communities.
• Collaborate with an Advanced Practice Clinician at each site.
• Deliver patient-centered care to older adults and post-acute patients.
• Support quality outcomes with a team-based care approach.
