We seek a medical leader with MD/DO, board certification in geriatrics or related fields, and an active license.

Qualifications

• MD/DO degree and board certification in relevant specialty.
• Active, unrestricted medical license in the state of practice.
• DEA registration and current BLS/ACLS certification.
• Proficiency with EMR systems and timely documentation.

Responsibilities

• Build Meaningful Relationships: manage care for residents in assigned facilities.
• Deliver Comprehensive Care: full assessments, chronic condition management, preventive and acute care.
• Stay Connected with Residents: conduct routine rounds to ensure continuity of care.
• Lead Care Collaboration: work with facility staff and families on personalized care plans.
• Ensure Excellence & Compliance: maintain EMR documentation and follow regulatory standards.
• Educate & Empower: guide residents and families on preventive care and resources.
• Drive Quality Outcomes: participate in quality initiatives and use data to improve care.
