Job summary

Bedrock Health Care Center in Wisconsin is seeking a Physical Therapist to join our team. You will evaluate and treat residents, document care, and participate in interdisciplinary meetings to help patients regain independence and maintain dignity in a skilled nursing setting.

You will implement individualized treatment plans, ensure timely documentation, and collaborate with physicians and staff while adhering to facility policies. A $10,000 sign‑on bonus is offered to eligible candidates.

Qualifications

• Excellent verbal and written communication and organizational skills.
• Strong reasoning ability and basic computer skills (Word/Excel).
• Doctorate of Physical Education and active Physical Therapist licensure.

Responsibilities

• Evaluate and document evaluations of referred residents per facility policies and procedures.
• Provide treatment to help residents attain the highest level of function possible.
• Recommend and provide appropriate treatment, preventative measures and adaptive equipment to prevent decline.
• Maintain timely resident reports as required by regulatory agencies.
• Observe infection control procedures.
• Complete documentation related to evaluations, treatment plans, MDS assessments, and care plans.
