Job summary

BoldAge PACE seeks a dedicated Physical Therapist to join our Fresno IDT and deliver quality therapy services to participants living at home. You will assess needs, plan and provide treatments, and educate participants and caregivers to maintain independence and safety in daily activities.

The role emphasizes collaboration with the interdisciplinary team, adherence to standards, and ongoing professional growth within a mission-driven organization serving seniors.

Qualifications

• Bilingual and fluent in Hmong preferred.
• Advanced degree in Physical Therapy from an accredited school.
• Current license to practice Physical Therapy in the state.
• Current driver’s license and proof of auto insurance.
• Two (2) years of experience on an interdisciplinary team in acute care or community settings preferred.
• One year experience with frail or elderly populations preferred; training provided if needed.

Responsibilities

• Provides high quality care as a member of the PACE interdisciplinary team (IDT).
• Performs in-person assessments for participants at initial enrollment and ongoing as determined by the IDT.
• Assesses the need for adaptive equipment and home safety modifications.
• Plans and implements physical therapy treatment programs aligning with standards.
• Documents evaluation, plan and treatment; communicates with the IDT.
• Educates participants and families on procedures and home programs.
