Job summary

EmpowerMe Wellness is seeking licensed therapists (PT/OT/SLP) to support seniors in partner senior living communities as part of EmpowerMe 360. You’ll provide therapy and wellness services, assess needs, and craft personalized care plans.

You’ll collaborate with a multidisciplinary team, conduct care coordination and telehealth check-ins, and help residents maximize independence and safety while delivering compassionate care.

Qualifications

• Current professional license in your therapy discipline (PT, PTA, OT, COTA, or SLP).
• Passion for empowering seniors to live their best lives.
• Strong clinical skills with a collaborative spirit.
• Love for teamwork and community engagement.

Responsibilities

• Provide therapy and wellness services for older adults.
• Conduct assessments, create personalized care plans, and deliver evidence-based interventions as part of a collaborative team.
• Support care coordination with wellness check-ins, caregiver communication, and telehealth support.
• Maintain professional and ethical standards in line with company initiatives.
• Contribute to program development and continuous quality improvement.
