Ascend Medical is seeking a compassionate and experienced Nurse Practitioner or Physician Assistant to provide relationship‑centered care at premier Senior Care communities in Sugar Hill and Grayson, GA. This part‑time role is ideal for a provider who is passionate about geriatrics and values quality time with patients over quantity.

You’ll be supported by a dedicated clinical care team, innovative technology, and full operational support so you can focus on delivering exceptional care and building trusted relationships.

Locations

Sugar Hill – 1.5 days (12 hours) per week.

Grayson – 1 day (8 hours) per week.
