Job summary

Ascend Medical is seeking a compassionate Nurse Practitioner or Physician Assistant to deliver relationship-based care for older adults in premier assisted living facilities in Loganville and surrounding areas. This full-time role emphasizes geriatrics, with four on-site days and one admin day, supported by a dedicated clinical team and robust training.

Qualified providers will have GA licensure, an active DEA license, and a minimum of 3 years in geriatrics, hospice, or home health.

Qualifications

• Licensed NP or PA in Georgia (unrestricted, non-probationary).
• Master’s degree from an accredited program.
• Minimum of 3 years of clinical experience, with strong preference for geriatric, hospice, or home health background.
• Active DEA license (or eligibility to obtain).
• Availability to work on-site in Loganville and surrounding area.

Responsibilities

• Provide compassionate, relationship-based care to older adults in an assisted living environment.
• Build trust with patients, families, and facility staff through clear communication and consistent presence.
• Manage chronic conditions, address acute concerns, and tailor care plans to the unique goals of each patient.
