Job summary

Curana Health is seeking Nurse Practitioners or Physician Assistants to deliver proactive primary care directly in senior living communities and skilled nursing facilities. You will build patient relationships, develop care plans, and work with physicians and care teams to optimize outcomes.

Autonomy is emphasized, with support from operational leaders and advanced technology. Travel to assigned facilities is required to provide high-quality care wherever residents live.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations.
• Current board certification in nursing or physician assistant field.
• Masters degree in nursing or physician assistant studies.
• Strong leadership, interpersonal, and communication skills.

Responsibilities

• Deliver high-quality primary care across skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic conditions with preventative focus.
• Provide ongoing care management to improve quality of life and independence.
• Collaborate with physicians, care teams, and families to coordinate care plans.
• Maintain accurate EMR documentation and ensure regulatory compliance.
