You'll practice at the top of your license, partner with physicians and clinicians, and build meaningful relationships with residents and families. Travel to multiple facilities is required, supported by advanced technology and a mission-driven

Qualifications

• Masters in nursing or PA studies and state license with board certification.
• Strong leadership, communication, and teamwork skills.
• Willingness to travel to multiple facilities as needed.
• Excellent written and oral communication abilities.

Responsibilities

• Deliver high-quality primary care to residents in skilled nursing and related communities.
• Diagnose and manage acute and chronic conditions with preventative focus.
• Develop individualized treatment plans and maintain EMR documentation.
• Collaborate with physicians, staff, and families to coordinate care.
• Engage in care discussions and support clinical excellence initiatives.
