Job summary

Optum Home & Community Care, part of Optum, seeks an NP or PA to deliver care across facilities in Multnomah and Washington Counties, OR. The role is full‑time, travel‑required, with a 8:00am–5:00pm schedule Monday‑Friday.

Primary duties include conducting assessments, managing acute and chronic conditions, coordinating with the care team, and counseling patients and families. No on‑call, weekends, or holidays required.

Qualifications

• NP or PA graduate with licensing in the residing state and board certification.
• Active and unrestricted license in state of practice or ability to obtain by start date.
• DEA licensure or ability to obtain post‑hire per state regulations.
• Ability to gain a collaborative practice agreement, if applicable.
• Ability to perform physical tasks and travel to multiple facilities.

Responsibilities

• Conduct comprehensive assessments.
• Manage patients during acute or chronic illness episodes.
• Assist with short‑term sub‑acute rehabilitation care.
• Document accurately including ICD‑10 codes.
• Collaborate with interdisciplinary care teams.
• Engage in advanced care planning conversations.
• Educate patients and caregivers.
