Job summary

Medix is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to provide primary care across senior housing communities in the Phoenix area. This on-site, facility-based role offers direct patient impact with flexible hours and high autonomy.

The position supports residents in skilled nursing, assisted living, and independent living communities, with a focus on preventive care, long-term wellness, and reducing hospitalizations.

Qualifications

• Master's degree required (MSN or MPAS).
• Active Arizona NP or PA license with national board certification.
• Strong leadership, interpersonal, and communication skills.
• High level of punctuality, organization, and collaborative mindset.

Responsibilities

• Perform health assessments and develop care plans for residents in multiple living settings.
• Diagnose, treat, and manage acute and chronic conditions with preventive focus.
• Provide ongoing care management to improve quality of life and independence.
• Conduct facility rounds and build relationships with residents, families, and care teams.
• Coordinate care with physicians and other clinicians; ensure accurate EMR documentation.
