This full-time role emphasizes meaningful relationships with residents and a collaborative care environment. Ideal candidates will have active licenses and experience in long-term care.

The position offers a competitive salary of $150,000 - $200,000 per year along with benefits such as health coverage, 401K, and a sign-on bonus, allowing you to make a significant difference in seniors' lives.

Qualifications

• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license.
• State-specific license in good standing in Ohio.
• Minimum 6 months of experience in long-term care or a skilled nursing facility.

Responsabilités

• Provide primary care services to residents in skilled nursing and long-term care facilities.
• Perform physical exams and medical history assessments.
• Diagnose and treat chronic and acute health conditions.
• Order and interpret diagnostic tests and lab results.
• Develop and adjust individualized care plans.
