Job summary

Infinite Medical P.C. is hiring a Primary Care Nurse Practitioner/Physician’s Assistant to provide on-site care to residents in long-term care facilities in Dayton, OH. This is a full-time role focusing on meaningful patient relationships within a collaborative care team.

You will perform exams, manage chronic and acute conditions, order tests, and educate residents and families. Ohio license active is required; LTC experience preferred; competitive salary and benefits offered.

Qualifications

• Active and unrestricted NP/PA license (or physician license) required.
• State license in good standing in Ohio.
• Minimum 6 months of experience in long-term care or skilled nursing facility.
• Strong communication and clinical skills.

Responsibilities

• Provide primary care services to residents in skilled nursing and long-term care facilities
• Perform physical exams and medical history assessments
• Diagnose and treat chronic and acute health conditions
• Order and interpret diagnostic tests and lab results
• Develop and adjust individualized care plans
• Prescribe medications and monitor treatment effectiveness
• Collaborate with facility staff and interdisciplinary teams
• Educate patients and families about treatment options and care goals
• Ensure a safe, respectful, and supportive environment for residents
