Curana Health is hiring Nurse Practitioners and Physician Assistants to provide proactive, relationship-based primary care to seniors in skilled nursing, assisted living, and independent living communities. You will practice with autonomy, supported by physicians and advanced technology, and contribute to reducing hospitalizations and enhancing patient outcomes.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations and board certification
• Master's Degree in Nursing or Physician Assistant Studies
• Leadership, interpersonal and problem-solving skills preferred
• Excellent written and oral communication skills
• Punctuality, organizational skills, and ability to work in a team

Responsibilities

• Deliver high-quality primary care in skilled nursing and related settings
• Build meaningful patient relationships and manage ongoing care
• Collaborate with physicians, staff and specialists to coordinate care
• Maintain EMR documentation and ensure regulatory compliance
• Travel to and from assigned facilities as required
