Curana Health is seeking Nurse Practitioners or Physician Assistants to provide proactive, relationship-based primary care to seniors in skilled nursing, assisted living, and independent living communities. You’ll work with a supportive team of physicians and operations leaders, leverage advanced technology, and practice with a high degree of autonomy.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations and board certification.
• Masters degree in nursing or PA studies.
• Strong leadership, interpersonal, and problem-solving skills; excellent written and oral communication.

Responsabilités

• Deliver High-Quality Primary Care to residents in skilled nursing, assisted living, and independent living communities.
• Build meaningful patient relationships through proactive, relationship-based care.
• Collaborate across the care continuum with physicians, staff, and families to coordinate care plans.
• Drive clinical excellence by improving outcomes and reducing avoidable hospitalizations.
• Maintain accurate EMR documentation and ensure regulatory compliance.
