Curana Health, Inc. is seeking a Nurse Practitioner or Physician Assistant to provide proactive, relationship-based primary care to seniors in skilled nursing and senior living communities.

You will work with experienced physicians and care teams to deliver autonomous, high-quality care across Curana’s network. Qualifications

• Valid nurse practitioner or physician assistant license in the state of operations.
• Current board certification in NP/PA.
• Masters Degree in Nursing or Physician Assistant Studies.
• Willingness to travel to one or more assigned facilities across Curana Health network.

Responsibilities

• Deliver high-quality primary care to residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic medical conditions with emphasis on preventative care and long-term wellness.
• Develop individualized care plans and build meaningful patient relationships with residents and families.
• Collaborate with physicians, clinical staff, facility leadership, specialists, and family members to coordinate comprehensive care plans.
• Maintain accurate EMR documentation and ensure compliance with regulatory standards.
• Contribute to interdisciplinary care discussions and support clinical excellence initiatives.
