Curana Health, Inc. is seeking a Nurse Practitioner or Physician Assistant to deliver high-quality primary care to seniors in skilled nursing, assisted living, and independent living communities. You will work with physicians and care teams, developing treatment plans and managing care across multiple facilities.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations.
• Masters Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Must possess excellent written and oral communication skills.
• Must possess traits of punctuality, organizational skills, and ability to function within a team.

Responsabilités

• Perform comprehensive health assessments and develop individualized treatment plans for residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic medical conditions with a focus on preventative care and long-term wellness.
• Provide ongoing care management that improves quality of life while helping residents remain healthier and more independent.
• Build meaningful patient relationships through regular facility rounds with residents, families, and care teams.
• Collaborate with physicians, clinical staff, facility leadership, specialists, and family members to coordinate comprehensive care plans.
• Maintain accurate and timely documentation within the EMR and ensure compliance with regulatory standards.
