Job summary

Curana Health seeks a compassionate Nurse Practitioner or Physician Assistant to deliver high-quality primary care in senior living facilities. You will provide direct patient care, manage chronic conditions, and coordinate with other healthcare providers to improve health outcomes for seniors.

The role involves travel to assigned facilities and requires a valid NP/PA license and board certification, with strong communication and teamwork skills.

Qualifications

• NP or PA with state license and board certification.
• Master's degree required (Nursing or PA).
• Demonstrated leadership, interpersonal and problem-solving skills.

Responsibilities

• Provide direct patient care and management for residents in senior living facilities.
• Perform comprehensive assessments and develop treatment plans.
• Coordinate care with facility staff and other providers.
• Educate residents and families on wellness and disease management.
• Maintain EMR documentation accurately and in compliance with standards.
• Participate in quality improvement initiatives to enhance care.
