Job summary

Curana Health, Inc. is seeking Nurse Practitioners or Physician Assistants to lead primary care for seniors in skilled nursing, assisted living, and independent living communities.

You will deliver proactive care directly at the residents' locations, building trusted relationships and improving outcomes. You will practice with a high degree of autonomy while supported by physicians, operational leaders, and advanced technology.

Qualifications

• Licensed to practice as a nurse practitioner or physician assistant in the operating state.
• Master's degree in nursing or physician assistant studies with current board certification.
• Strong leadership, communication, and collaborative skills.

Responsibilities

• Deliver high‑quality primary care with comprehensive assessments and individualized treatment plans.
• Build meaningful patient relationships through ongoing care and education.
• Collaborate across the care continuum with physicians, staff, and families.
• Drive clinical excellence, improve outcomes, and support regulatory compliance.
• Maintain accurate EMR documentation and ensure data integrity.
