Job summary

Your Health in Augusta, GA is seeking a Senior Solutions Advisor to assist with patient needs in the Augusta area. The SSA acts as a trusted resource for patients and families navigating aging, chronic illness, and care transitions, connecting them to healthcare and private caregiving options.

This role travels to patients’ homes and facilities daily, is full-time and salary-based (8:00AM-5:00PM, Monday-Friday).

Qualifications

Bachelor's degree in social work or related field preferred.
Experience in case management or healthcare navigation preferred.
Strong interpersonal and communication skills.
Valid driver's license and reliable transportation.
Ability to travel between offices and facilities.

Responsibilities

Meet with patients and families to assess long-term care needs and social support challenges.
Provide consultative guidance on living arrangements, safety concerns, and available Your Health services.
Serve as the regional point of contact for onboarding new patients.
Maintain knowledge of local senior care resources (assisted living, home care, SNFs, etc.).
Serve as a liaison between patients, families, and resources to ensure coordination.
Educate community professionals and caregivers through outreach and training.
Timely communication with new patients in the assigned Care Group.
Support care transitions by working with Transitional Care Providers and care teams.
Assist families in accessing care management and hospice services.
Document activities for billing structures (CCM, ACP).
Participate in community events to promote SSA services.
Collaborate to support market expansion through relationships and referrals.
