A Silver Lining: Aging Advocacy & Care Management is seeking a dedicated Aging Life Care Manager for a full-time, on-site role in Pinellas County, Florida. Qualifications

• Experience in case management: assessment, care planning, service coordination, oversight.
• Background in Elder Care, understanding aging-related needs and senior care options.
• Experience in Social Services supporting families through complex situations.
• Familiarity with healthcare systems, medical terminology, and clinical providers.
• Ability to provide training and education to clients, families, and caregivers.
• Bachelor’s or Master’s degree in related field preferred.
• Licensure or certification beneficial (e.g., LCSW, RN, CCM).
• Strong communication, advocacy, and conflict-resolution skills.
• Valid driver’s license and willingness to travel across Pinellas County.
• Proficiency with documentation and basic office/EHR systems.

Responsibilities

• Conduct comprehensive assessments of clients' medical, psychosocial, and environmental needs.
• Develop individualized care plans and coordinate services with providers and families.
• Communicate regularly with families and healthcare professionals.
• Respond to crises and advocate for clients within healthcare and community systems.
• Monitor client status, arrange and attend appointments, and facilitate family meetings.
• Maintain accurate documentation in accordance with professional and regulatory standards.
