  • Remote role. Candidate must reside in Florida in Hardee County, Highlands County, Hillsborough County, Polk County, or surrounding areas. Must have four years or more of Pediatric experience and LPN licensure.

Position Purpose: Assesses, plans, implements, and coordinates care management activities based on member needs to provide quality, cost-effective healthcare outcomes. Develops or contributes to the development of a personalized care plan/service plan for members and educates members and their families/caregivers on available services and benefits to improve healthcare access and quality care through advocacy and coordination.

  • Develops or contributes to ongoing care plans and identifies providers, specialists, and community resources needed for care.
  • Coordinates and manages communication between the member, family/caregivers, and care providers to ensure timely access to services.
  • Monitors progress towards care plan goals and collaborates with healthcare providers for revisions or further evaluation.
  • Provides psychosocial and resource support to members/caregivers and care managers, helping access local resources such as employment, housing, food, and other services.
  • Identifies barriers to care and implements appropriate interventions for members with higher care needs.
  • Reviews referrals and assessments to develop suitable care plans for members with complex needs.
  • Collaborates with healthcare partners to ensure member needs are met and revise care plans as necessary.
  • Documents all member information and activities to ensure compliance with regulations.
  • May perform on-site visits and provide education on procedures, care options, and benefits.
  • Serves as a Subject Matter Expert (SME) and may serve as a preceptor for new staff.
  • Performs other duties as assigned to meet business needs.

Education/Experience: Requires a Bachelor’s degree and 4–6 years of related experience. Clinical licensure as a Nurse is preferred. Equivalent experience may be considered.

License/Certification:

  • Current state clinical license preferred.
  • For CMS Florida licensed practical nurses with pediatric experience or Master’s in social work with pediatric care experience. Up to 80% local travel may be required.

Pay Range: $26.50 - $47.59 per hour
