SeaStar Senior Services, LLC in Eugene, Oregon is seeking a professional Care Manager to coordinate care for seniors living at home. The role starts at 5-10 hours per week with potential to grow as our client base expands, and includes after-hours on-call.

Responsibilities include assessments, care plans, coordinating medical appointments, arranging services, and serving as liaison between families and providers. Part-time, in-person position with on-call rotation and benefits.

Qualifications

• Registered Nurse license or MSW degree required.
• Experience with seniors and adults with disabilities preferred.
• Excellent driving record and own car.

Responsabilités

• Perform comprehensive assessments of health history, family dynamics, and environmental needs.
• Create and update care plans and coordinate access to services.
• Coordinate medical appointments and medications; obtain needed services.
