Job summary

Sycamores seeks a Lead Care Manager Supervisor Enhanced Care Management to oversee day-to-day operations and supervise a team of Lead Care Managers. The role ensures services align with contracts, policies, and the agency’s treatment philosophy.

Requirements include a Bachelor’s degree, at least 2 years of supervisory experience, and ability to travel between client locations. A valid CA driver license and vehicle are required.

Qualifications

• Bachelor’s degree in a related field.
• Minimum of 2 years prior supervisory experience in a related field.
• Experience leading projects (team lead) can also be included.
• Experience working with populations of focus: homelessness, high-needs, and at-risk individuals or those with serious mental health/substance use needs.
• Knowledge of Whole Person Care approach and integration of physical health, mental health, and substance use.
• Maintains all required licenses and certifications.
• Valid California Driver License and driving record acceptable to agency.

Responsibilities

• Oversee day-to-day administration of programming activities.
• Provide direct supervision to a team of Lead Care Managers.
• Implement program practices aligned with organizational policies, contracts, and treatment philosophy.
• Perform direct service tasks as needed to support departmental needs.
